29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  a. On or about 03/29/2021, at the job site - employees conducting roofing work from the first story roof while other employees were working from the second story roof were exposed to struck-by hazards to their heads, in that, they were not using head protection.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a. On or about 03/29/2021, at the job site - employees conducting roofing work from a 8:12 pitched roof were exposed to an 18 foot 2 inch fall hazard from the edge of the second story roof to the ground, and an 8 foot 8 inch fall hazard from the edge of the first story 6:12 pitched roof to the ground, in that, they were working without fall protection.
