29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents: New townhome construction on Lots #60-#66 Duval Drive, Alpharetta, GA 30009, Roof: On or about 04/09/2021, the employer exposed employees to struck-by hazards while operating Hitachi Framing Nail Guns, in that protective eyewear was not provided to employees operating these tools at 130 PSI while engaged in framing and sheathing activities.

29 CFR 1926.501(b)(13):Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501(b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502: New townhome construction on Lots #60-#66 Duval Drive, Alpharetta, GA 30009, Roof: On or about 04/09/2021, the employer exposed employees to fall hazards of 21-feet, 7-inches to the slower level, in that a fall protection system was not provided to employees engaged in framing and sheathing work on a 6/12 pitched roof.

29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: New townhome construction on Lots #60-#66 Duval Drive, Alpharetta, GA 30009, Garage: On or about 04/09/2021, the employer failed to inspect electrical equipment including extension cords and electric air compressor, in that flexible cords were missing ground pins and the Makita (Model 5007F) 7-1/4-inch Circular Saw used had a damaged strain relief on the end of the cord which exposed the internal insulated wiring.
