29 CFR  1910.134(f)(2):Employee(s) using a tight-fitting facepiece respirator were not annually fit tested:  Drilling Site: On or about April 8, 2021, the  employer did not ensure employees engaged in drilling operations in a Hydrogen Sulfide field and were prepared to conduct a rescue had completed fit testing on the rescue respirators.  This condition exposed employees to health hazards.

29 CFR  1910.304(g)(5):The path to ground from circuits, equipment, and enclosures was not permanent, continuous, and effective:  Drilling Site: On or about April 08, 2021, the employer did not ensure the Lube Roll-Off trailer and crew quarters were grounded which are located on a Hydrogen Sulfide field and are powered by portable power.  This condition exposed employees to fire and electrical shock hazards.

29 CFR  1910.1200(h)(3)(iii): Employee training did not include the measures employees can take to protect themselves from chemical hazards, including specific procedures the employer had implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures and personal protective equipment to be used:  Drilling Site: On or about April 8, 2021, the employer did not ensure employees engaged in drilling activities on a Hydrogen Sulfide field were trained to conduct a rescue with a Self Contained Breathing Apparatus (SCBA).  This condition exposed employees to health hazards.
