29 CFR  1910.132(d)(2):  The employer did not verify, through a written certification, that the required workplace hazard assessment had been performed:  (a)  Western Sugar Cooperative, 18317 CO-144, Fort Morgan, CO, 80701:  On or about April 22, 2021, and at times prior, the employer did not ensure its employees and employees of Advance Services, Inc., temporary labor agency, were protected by personal protective equipment where necessary by certifying in writing a workplace hazard assessment.  Employees are exposed to at least but not limited to splashing and burn hazards, which could be mitigated by PPE, in the facility.

29 CFR  1910.147(c)(7)(iv):  The employer did not certify that employee training had been accomplished and kept up to date:  (a)  Western Sugar Cooperative, 18317 CO-144, Fort Morgan, CO, 80701:  On or about April 22, 2021, and at times prior, the employer did not certify that an employee of ASI, temporary staffing agency, was properly trained as an authorized employee to lockout 4th floor sugar filters for servicing and maintenance exposing the employee to burn hazards.
