29 CFR  1910.179(h)(4):  Hooks did not meet the manufacturer's recommendations:  a) Western Sugar, Beat Wash Unit: On or about March 13th, 2024, and at times prior, employees were exposed to struck by hazards while operating a hoist to pull pumps from the recovery pool with a hoist hook containing a  broken throat safety latch.

29 CFR  1910.253(b)(2)(ii):Inside of buildings, cylinders were not stored in a well-protected, well-ventilated, dry location, at least 20 feet (6.1 m) from highly combustible materials such as oil or excelsior:  a) Western Sugar, Causette Mixer Room: On or about March 13th, 2024, and at times prior, employees were exposed to fire and secondary explosion hazards while storing tools and retrieving equipment near an oxidizer (Induclor - calcium hypochlorite) resting on the floor approximately four feet away from an oxygen and acetylene compressed gas cylinders.

29 CFR 1910.29(b)(2): The employer did not ensure that guardrail systems met the requirement that  midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members that are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high.   a. Western Sugar, Dorr-Oliver Room: On or about March 13th, 2024, and at times prior, employees were exposed to fall hazards while storing equipment and fixing handrails. The employer failed to ensure that the guardrail system in the area has a proper midrail.
