The exposure was derived from a sample taken over 604 minutes.  b) On or about June 4, 2021 at Plant 4 - employees exposed to noise above the 10-hour action level did not receive a baseline audiogram within one year of the employee's first exposure. Noise sampling performed on an employee working on line 1 ready to eat (RTE) resulted in noise levels at a time-weighted average (TWA) of 91.1 dBA, which exceeded the 10-hour action level of 83.4 dBA. The exposure was derived from a sample taken over 559 minutes.  c) On or about June 4, 2021 at Plant 4 - employees exposed to noise above the 10-hour action level did not receive a baseline audiogram within one year of the employee's first exposure. Noise sampling performed on an employee working at the platform on line 2 separating chicken on the conveyor resulted in noise levels at a time-weighted average (TWA) of 91.9 dBA, which exceeded the 10-hour action level of 83.4 dBA. The exposure was derived from a sample taken over 588 minutes.  d) On or about June 4, 2021 at Plant 4 -  employees exposed to noise above the 10-hour action level did not receive a baseline audiogram within one year of the employee's first exposure. Noise sampling performed on an employee working at the on line 3 marination resulted in noise levels at a time-weighted average (TWA) of 91 dBA, which exceeded the 10-hour action level of 83.4 dBA. The exposure was derived from a sample taken over 572 minutes.  e) On or about June 4, 2021 at Plant 4 - employees exposed to noise above the 10-hour action level did not receive a baseline audiogram within one year of the employee's first exposure. Noise sampling performed on an employee working on line 3 marination resulted in noise levels at a time-weighted average (TWA) of 88.1 dBA, which exceeded the 10-hour action level of 83.4 dBA. The exposure was derived from a sample taken over 568 minutes.  f) On or about June 4, 2021 at Plant 4 - employees exposed to noise above the 10-hour action level did not receive a baseline audiogram within one year of the employee's first exposure. Noise sampling performed on an employee working in line 2 bagging area resulted in noise levels at a time-weighted average (TWA) of 89.9 dBA, which exceeded the 10-hour action level of 83.4 dBA. The exposure was derived from a sample taken over 558 minutes.  g) On or about June 4, 2021 at Plant 4 - employees exposed to noise above the 10-hour action level did not receive a baseline audiogram within one year of the employee's first exposure. Noise sampling performed on an employee working at the platform on line 2 and line 1 in RTE resulted in noise levels at a time-weighted average (TWA) of 87.0 dBA, which exceeded the 10-hour action level of 83.4 dBA. The exposure was derived from a sample taken over 601 minutes.  i) On or about June 4, 2021 at Plant 4 - employees exposed to noise above the 10-hour action level did not receive adequate training. Noise sampling performed on an employee working on line 3 bagging/packing area resulted in noise levels at a time-weighted average (TWA) of 91.3 dBA which exceeded the 10-hour action level of 83.4 dBA. The exposure was derived from a sample taken over 596 minutes.  j) On or about June 4, 2021 at Plant 4 - employees exposed to noise above the 10-hour action level did not receive adequate training. Noise sampling performed on an employee working on line 2 batter station resulted in noise levels of 91.0 dBA which exceeded the 10-hour action level of 83.4 dBA. The exposure was derived from a sample taken over 235 minutes which represents the employee's 10 hours work day.  k) On or about June 4, 2021 at Plant 4 - employees exposed to noise above the 10-hour action level did not receive adequate training. Noise sampling performed on an employee operating the bagger on line 1 RTE resulted in noise levels at a time-weighted average (TWA) of 88.9 dBA, which exceeded the 10-hour action level of 83.4 dBA. The exposure was derived from a sample taken over 332 minutes.  o) On or about June 4, 2021 at Plant 4 - employees exposed to noise above the 10-hour action level did not receive adequate training. Noise sampling performed on an employee adding flour on line 2 resulted in noise levels at a time-weighted average (TWA) of 93.9 dBA, which exceeded the 10-hour action level of 83.4 dBA. The exposure was derived from a sample taken over 607 minutes.  p) On or about June 4, 2021 at Plant 4 - employees exposed to noise above the 10-hour action level did not receive adequate training.
