An authorized maintenance employee was required to work on equipment and machinery and did not receive authorized employee training on lockout tagout. Lockout tagout training was not provided on the recognition of the hazardous energy sources, the type and magnitude of the energy available in the workplace, and the methods and means necessary for energy isolation and control.

An extension cord was being used to power a relocatable power tap which was being used to power a label machine. Relocatable power taps are listed and labelled to be plugged directly into an outlet.
