29 CFR 1910.147(c)(4)(i):  Procedures were not utilized for the control of potentially hazardous energy when employees were engaged in the activities covered by this section.  At the worksite located at 6900 Stillwell Street, Kansas City, Missouri:  Employees were exposed to struck by hazards when diagnosing and servicing a sensor issue on the Line 2 Zechetti.  Employees bypassed the interlock on the door to the cage which houses the pallet dispenser when realigning the sensors causing the centering device to come down on an employee, leading to cracked ribs.

29 CFR 1910.147(c)(6)(i):  The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  At the worksite located at 6900 Stillwell Street, Kansas City, Missouri:  Missouri:  The employer did not conduct periodic inspections of the lockout/tagout procedures at least annually.  Employees were exposed to struck by hazards when diagnosing and servicing a sensor issue on the Line 2 Zechetti prior to locking out and tagging out the machine.
