North Side of Manufacturing Building: On or about 5/27/2021, the employer exposed employees to caught-in and amputation hazards, in that the Groove Roller Machine (Brand: Niagara No. 192) was not guarded at the point of operation while fabricating the metal sections for fire dampers.

29 CFR  1910.303(b)(1): Electrical equipment was not free from recognized hazards that were likely to cause death or serious physical harm to employees, in that the employer did not ensure the safety of the equipment pursuant to the considerations in sections (i) through (viii) of 29 CR 1910.303(b)(1):  a.  CRD Frame Machine area: On or about 5/27/2021, the employer exposed employees to electric shock hazards, in that an electrical outlet pendant was pinned in the metal enclosure of the frame machine where grease and rotating end shafts were located while fabricating the metal sections for fire dampers.
