29 CFR  1910.212(a)(3)(ii):Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s):  a)  Lloyd Industries, Inc. Roll Former "A" Line -  On or about December 13, 2021, the point of operation for the Roll Former Machine was not guarded to prevent employees from having their fingers in the danger zone during operation.    b)  Lloyd Industries, Inc. Roll Former "CRD" Line - On or about December 13, 2021, the point of operation for the Roll Former Machine was not guarded to prevent employees from having their fingers in the danger zone during operation.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photograph or video evidence of abatement or other written records.

29 CFR  1910.219(f)(3):Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed:  a)  Lloyd industries, Inc - On or about December 13, 2021, the sprocket and chain driving the "CRD" line roll former machine was not guarded to protect employees from rotating parts.  Abatement certification required within 10 days after abatement date.   The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1910.303(g)(1)(ii): The required working space about electric equipment rated 600 volts, nominal, or less to ground, was used for storage:   a)  Lloyd Industries, Inc. - On or about December 13, 2021, an energized electrical panel that is required to be accessed for examination, servicing, maintenance or emergency was obstructed by stored materials and had a clear front working space of approximately 4".    Abatement certification required within 10 days after abatement date.   The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.

OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees are operating a Spot Welder with an unprotected foot pedal actuator, exposing employees to unexpected machine operation.  a)  Lloyd Industries, Inc. - On or about December 13, 2021, the foot pedal actuator for the Welding Industries, Rocker Type Spot Welder was not guarded to prevent accidental operation of the electrodes with the employee's hands in the point of operation.  ABATEMENT NOTE:  Among other methods, one feasible and acceptable abatement method to correct this hazard is to guard the foot pedal in accordance with ANSI B11.19,  8.8.1.3, Single control safeguarding devices,  The single control device shall be designed and constructed to protect the control against unintended or inadvertent actuation.  Abatement certification required within 10 days after abatement date.   The certification shall include a statement that abatement is complete, date, and method of abatement, and states employees and their representatives were informed of this abatement.
