29 CFR  1926.651(j)(1): Adequate protection was not provided to protect employees from loose rock or soil that could pose a hazard by falling or rolling from an excavation face:   On or about June 24, 2021, near the intersection of FM 1960 Rd E and Atascocita Rd in Humble, TX, employees working in an excavation were exposed to struck by hazards from loose rock which posed a hazard.

29 CFR  1926.652(a)(1):Each employee in an excavation was not protected from cave-ins by an adequate protective system designed in accordance with 29 CFR 1926.652 (b) or (c).  On or about June 24, 2021, near the intersection of Near Intersection of FM 1960 Rd E and Atascocita Rd in Humble, TX, three employees were exposed to struck-by and crushed-by hazards when installing water lines in an excavation exceeding a depth of 5 feet without adequate protection from cave-ins.
