29 CFR  1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials, and equipment to be made by a competent person(s):  On or about June 17, 2021, at the location of 3060 Eggers Ave, Cleveland Ohio, 44105, near the exterior of the #2 Steel Producing department, East wall of the scrap yard, near the North End of the BOF (Basic Oxygen Furnace), the employer failed to maintain a program that included frequent inspections made by a competent person, able to identify and convey hazards, such as piping which contained hazardous or dangerous material when employees of Kelley Steel Erectors, Inc. were hired to remove piping (fuel supply and return line) on the exterior of the building, exposing employees to fire and explosion hazards.

When the presence of any such substances was apparent or suspected, the employer did not perform testing and purging to eliminate the hazard before demolition was started:  On or about June 17, 2021 the employer did not effectively communicate to the demolition contractor the potential hazards related to the presence of flammable liquids in the fuel supply and return lines that were being removed.�
