29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:  (a) On or about 06/24/2021 - 1902 Woodlands Industrial Drive, Trussville, AL, the employer did not an amputation injury that occurred on 06/24/2021 until 06/29/2021.
