29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  The employer is failing to protect employees from amputation hazards.  This was most recently documented at 36955 McCormick Drive, Lincoln, Nebraska on July 8, 2021. The employer failed to ensure that the timing belt slitter was guarded in such a manner as to prevent operators from reaching into the point of operation while the machine was being operated.

29 CFR  1910.147(c)(1):The employer did not establish an energy control program consisting of energy control procedures, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, start up or release of stored energy could occur and cause injury, the machine or equipment would be isolated and rendered inoperable:  The employer is failing to protect employees from electrical shock, amputations, struck by, and caught in between hazards.   This was most recently documented at 36955 McCormick Drive, Lincoln, Nebraska on July 8, 2021. The employer has not developed a program regarding lockout/tagout of equipment.  This would include written periodic inspections, training, abandoned lock procedures, and entry of outside personnel.
