29 CFR  1926.501(b)(12):  Each employee engaged in the erection of precast concrete members (including, but not limited to the erection of wall panels, columns, beams and floor and roof "tees") and related operations such as grouting of precast concrete members, who is 6 feet (1.8m) or more above lower levels were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems.   On or about August 10, 2021, the employer did not ensure the horizontal lifeline was properly secured to the anchor.  Employees were installing a concrete beam at a height greater than six (6) feet and this exposed the employees to a fall hazard to the concrete ground and construction debris below.

29 CFR  1926.502(d)(8): Horizontal lifelines were not designed, installed, and used under the supervision of a qualified person, as part of a complete personal fall arrest system, which maintained a safety factor of at least two.  On or about August 10, 2021, the employer did not ensure the horizontal lifeline was installed and used under the supervision of a qualified person.  This exposed an employee in use of the horizontal lifeline to a fall hazard.

29 CFR  1926.503(a)(2)(ii): The employer did not ensure that each employee, as necessary, had been trained by a competent person in the correct procedures for erecting, maintaining, disassembling and inspecting the fall protection systems to be used:   On or about August 10, 2021, employees were in use of a horizontal life line system that was erected and disassembled by employees that were not trained in the correct procedures for erecting, maintaining, inspecting, and disassembling the system.  This exposed the employees to a fall hazard.
