29 CFR 1910.305(b)(2)(i):Each outlet box in completed installations did not have a cover, faceplate, or fixture canopy: a) Line 4,on floor, adjacent to hydraulic lift. On or about September 1, 2021, the outlet box used to power the hydraulic lift was damaged. The damage resulted in the cover being displaced and not effectively covering the inner wires of the outlet.

29 CFR 1910.303(b)(7): Mechanical execution of work. Electric equipment shall be installed in a neat and workmanlike manner. a) Line 4,on floor, adjacent to hydraulic lift. On or about September 1, 2021, the outlet box used to power the hydraulic lift was damaged. The damage resulted in the cover being displaced and not effectively covering the inner wires of the outlet. An attempted repair to the damaged outlet box was made with duct tape which did not effectively resolve the condition.

On or about September 1, 2021, the outlet box used to power the hydraulic lift was damaged. The damage resulted in the cover being displaced and not effectively covering the inner wires of the outlet. Sources of the damage included but were not limited to the hydraulic lift.
