Serious 29 C.F.R. 1926.964(b)4)(i): Before employees install lines parallel to existing energized lines, the employer shall make a determination of the approximate voltage to be induced in the new lines, or work shall proceed on the assumption that the induced voltage is hazardous. On or about October 1, 2021, at 810 Charlotte Avenue FPL substation, West Palm Beach, Florida, employees installing lines parallel to existing energized lines did not proceed on the assumption that induced voltage was hazardous.

29 CFR 1926.964(c)(13): The employer shall ensure that employees maintain the minimum approach distances, established by the employer under � 1926.960(c)(1)(i), from all grounded objects and from lines and equipment at a potential different from that to which the live-line barehand equipment is bonded, unless insulating guards cover such grounded objects and other lines and equipment: On or about 10/01/2021, at 810 Charlotte Avenue FPL substation, West Palm Beach, Florida, the employer failed to maintain the minimum approach distance to 3-phase 150K vertical overhead powerlines while cutting a powerline from an aerial lift bucket approximately 8-feet from energized powerline.
