29 CFR  1926.100(a): Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, was not protected by protective helmets.   a-80 Washington Ave, Suffern NY 10901: An employee operating a material hoist was exposed to overhead impact hazards in which roof shingle bundles were being conveyed to the roof as a means of head protection had not been utilized. Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels was not protected by guardrail systems, safety net system, or personal fall arrest system:   a-80 Washington Ave, Suffern NY 10901:  Employees engaged in roofing activities on a 7 on 12 pitched roof were exposed to fall hazards of approximately 30ft to the asphalt below in which a means of fall protection had not been utilized. Condition observed on or about 10/25/2021
