29 CFR  1910.67(c)(2)(v):(v) A personal fall arrest or travel restraint system that meets the requirements in 29 CFR part 1910, subpart I, were not worn and attached to the boom or basket when working from an aerial lift.  (a) Fishing Vessel (FV) Patricia L - On or about November 16, 2021 and at times prior thereto, employees were exposed to being ejected and falling approximately 15' from aerial work platform(s) without wearing personal fall arrest or fall restraint.  OSHA Instruction - Directive Number: CPL 02-00-162 - Effective Date: 05/22/2019 - Subject: Shipyard Employment "Tool Bag" Directive pg. 23 - General Industry Standard 1910.67 "applies on vessels and on shore for vehicle-mounted elevating & rotating work platforms."
