29 CFR  1926.451(c)(2)(v): The employer used forklifts  to support scaffold platforms without ensuring that the entire platform was attached to the fork and that the forklift was not moved horizontally while the platform was occupied:    (a) Dixey Land Energy at Customer Warehouse 902 Strasburg Pike Strasburg PA - The employer allowed an employee on an elevated surfaces of a wooden skid that was not attached/secured to the forklift, on or about December 10, 2021.   Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1926.451(g)(1):Employees on scaffolds more than 10 feet (3.1 m) above a lower level were not protected from falling to that lower level by fall protection established in paragraphs (g)(1)(i)-(vii) of this section:  (a) Dixey Land Energy at Customer Warehouse 902 Strasburg Pike Strasburg PA - The employer did not provide fall protection for the employee elevated 12 to 15 feet on an elevated surfaces of a skid, on or about December 10, 2021.  Abatement certification required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
