29 CFR  1910.147(c)(6)(ii):The employer did not certify that periodic inspections of the energy control procedures were performed:  An examination of program elements determined that periodic inspections of energy control procedures were not accomplished and certified.  Specifically, reviews were not documented and certified to determine if listed steps were still adequate, if any changes had occurred, or if employees had acquired the knowledge needed to effectively isolate energy sources.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  At the workplace located at 1642 Crystal Avenue, Kansas City, Missouri 64126, an employee was exposed to caught-in and amputation hazards. An employee was exposed to a rotating part on the bottom of the west side rotary airlock. The rotating part was not covered or guarded adequately enough to prevent the employee from being caught-in machine.
