29 CFR 1926.1053(b)(22): Employees using ladders were carrying objects or loads that could cause the employee to lose balance and fall:    a) On or about February 10, 2022, employees were exposed to the hazard of falling from elevations up to approximately 16 feet when climbing a portable extension ladder while carrying materials and equipment to a roof deck landing surface.    In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).

29 CFR 1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system:          a) On or about February 10, 2022, employees were exposed to the hazard of falling approximately 16 feet while performing manual roofing work activities without the use of fall protection or a warning line and safety monitoring system.      Windward Roofing and Construction, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(10) which was contained in OSHA inspection number 1429793, citation 2, item 1, and was affirmed as a final order on September 4, 2019, with respect to a worksite located at 1810 5th Ave., River Grove, Illinois.    No abatement certification or documentation required for this violation.
