29 CFR  1910.23(b)(10):29 CFR 1910.23(b)(10): The employer did not ensure that any ladder with structural or other defects as immediately tagged "Dangerous: Do Not Use" or with similar language in accordance with � 1910.145 and removed from service until repaired in accordance with � 1910.22(d), or replaced.  a) Main Deck, Shore side of captive barge, 398 W Illinois Street, Memphis, TN.  On or about 23 March 2022, and at times prior to, workers were using a damaged 20 foot ladder with broken sides which was repaired with duct tape exposing employees to a fall hazard.

29 CFR  1910.184(i)(9)(iv): Synthetic web sling(s) with broken or worn stitches were not immediately removed from service:  a)  Main Deck, Shore side of captive barge, 398 W Illinois Street, Memphis, TN.  On or about 23 March 2022, and at times prior to, a synthetic web sling that was frayed and excessively worn was not removed from service exposing employee to struck-by and/or crushing hazards in the event of a sling failure.

29 CFR  1917.26(f):  A US Coast Guard approved 30 inch life ring, with at least 90 feet of line attached, was not available at readily accessible points at each waterside work area where the employees' work exposed them to the hazard of drowning.    a)  Main Deck, Shore side of captive barge, 398 W Illinois Street, Memphis, TN.  On or about 23 March 2022, and at times prior to,  coast guard approved 30 in. life ring did not have 90 feet of line attachment exposing workers to drowning due to inability to rescue workers.
