29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  (a)   This violation occurred on or about March 31, at the jobsite located at 2311 Willow Wood TRL, Terrell, TX; where employees were doing roofing work on a new residential single-family house and were exposed to head injuries where overhead hazard exists by not wearing hard hats.

Employees were exposed to eye injuries.

29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  This violation occurred on or about March 31, at the jobsite located at 2311 Willow Wood TRL, Terrell, TX, where employees were using extension ladders that did not extend 3-feet above the landing.  Employees were exposed to a fall hazard: (a)	Werner 24-feet extension ladder, model number D224-2 located at the front of the house. (b)	Louisville 24-feet extension ladder, model number W2328-20 located at the front of the house.
