29 CFR  1926.501(b)(4): Each employee on walking/working surfaces shall be protected from falling through holes (including skylights) more than 6 feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.    a) Auburn Community Hospital, 17 Lansing Street, Auburn, NY  13021, on or about 4/13/22:  Employees were demolishing the interior spaces on the 2nd and 3rd floors, where holes were exposed and ineffectively or not covered at all  Abatement documentation must be submitted for this item.

29 CFR  1926.501(c)(3): The employer did not barricade the area to which objects could fall, did not prohibit employees from entering the barricaded area, and did not keep objects that could fall far enough away from the edge of a higher level so that the     a) Auburn Community Hospital, 17 Lansing Street, Auburn, NY  13021, on or about 4/13/22:  Employees were demolishing the interior spaces on the 2nd and 3rd floors, where holes were exposed and rubble fell through from the 2nd floor to the first floor and into an office space where people were working.    Abatement documentation must be submitted for this item.
