29 CFR 1926.501(a)(2): The employer shall determine if the walking/working surfaces on which its employees are to work have the strength and structural integrity to support employees safely. Employees shall be allowed to work on those surfaces only when the surfaces have the requisite strength and structural integrity. a) Jobsite: 2010 W Girard Ave, Philadelphia, PA - On or about April 12, 2022, the employer did not ensure the walking/working surfaces inside the 4 story residential apartment building had the strength and structural integrity to support the weight of the employees working inside the building exposing the employees to fall hazards.

29 CFR 1926.502(i)(2): All other covers were not capable of supporting, without failure, at least twice the weight of employees, equipment, and materials that may be imposed on the cover at any one time. a) Jobsite: 2010 W Girard Ave, Philadelphia, PA - On or about April 12, 2022, the employer did not ensure the cover placed over the floor hole measuring 9 feet in width, and 14 feet 6 inches in length, was capable of supporting without failure at least twice the weight of employees, equipment, and materials that could have been imposed on it.

29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a) Jobsite: 2010 W Girard Ave, Philadelphia, PA - On or about April 12, 2022, the employer did not provide fall protection training to employees engaged in framing activities on a residential building approximately 40 feet above the ground level. The employees were not trained on how to recognize, evaluate, and mitigate fall hazards.
