NOX US, LLC - Fostoria, Ohio: The employer does not ensure that the tripwire cable, located on the inbound/front side of Mix Mill #2 in area C-1, is no more than 72 inches above the level where the operator stands and located within 2 inches of a vertical plane tangent to the roll.  This violation was most recently observed on May 4, 2022, when the tripwire cable was located past the front of the roll and at a height approximately 84 inches on one end of the mill roll and up to approximately 88.5 inches where the operator stands, exposing the operator to caught-in hazards.  c. NOX US, LLC - Fostoria, Ohio: The employer does not ensure that the tripwire cable, located on the outbound/back side of Mix Mill #2 in area C-1, is no more than 72 inches above the level where the operator stands and located within 2 inches of a vertical plane tangent to the roll.
