29 CFR  1910.29(f)(3) Surfaces. The employer did not ensure that handrails and stair rail systems are smooth-surfaced to protect employees from injury, such as punctures or lacerations, and to prevent catching or snagging of clothing.  (a) Eastside Ranger Station - Exterior staircase hand rails - On or about May 25, 2022 and at times prior thereto, employees were exposed to hand injuries such as: cuts, punctures and splinters while using the severely rotten wooden hand rails to transit the two outside steps.

29 CFR  1910.157(e)(2): Portable extinguishers were not visually inspected monthly.  (a) Ranger District - On or about May 5, 2022 and at times prior thereto, employees were exposed to fire and smoke inhalation in that the portable fire extinguishers throughout the ranger district were not inspected monthly to ensure that they were fully charged and available for emergency use.  The Forest Service, Mount Hood National Forest - Barlow Ranger District - was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(2), which was contained in OSHA inspection number 1261318, citation number 2, item number 6 and was affirmed as a final order on 3/20/2018, with respect to a workplace located at Barlow Ranger District, Clear Lake Lookout Fire Tower.

(a) On or about May 5, 2022 and at times prior thereto, employees were exposed to fire and smoke inhalation without portable fire extinguishers that were properly maintained and inspected.  The Forest Service - Mount Hood National Forest - was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.157(e)(3), which was contained in OSHA inspection number 1261318, notice number 3, item number 2 and was affirmed as a final order on 3/20/2018, with respect to a workplace located at Barlow Ranger District - Clear Lake Lookout Fire Tower.
