On July 5, 2022, in the maintenance shop, employees did not utilize the lock out tag out procedures when trying to identify and diagnose a hydraulic leak in the hopper of the garbage trucks, exposing employees to a struck-by and crushing hazard.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: a. On July 5, 2022, in the maintenance shop, the employer permitted the use of tools or other materials to intentionally defeat the safety sensor/interlock device on the side access door to the Hercules frontend loader garbage truck in order to diagnose hydraulic leaks contained within the compactor body, exposing employees to a crushing and struck-by hazard.
