Tonnard Manufacturing Corporation- on or about 6/28/22, and times prior thereto: Emergency medical services were not called to the facility for the treatment of life threatening injury.  The employer did not have designated personnel who were readily available to provide first aid at the site.  ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below).  This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

Tonnard Manufacturing Corporation- on or about 7/6/22, and times prior thereto: Portable multi-purpose dry chemical fire extinguishers were distributed throughout the facility for employees to use in the event of an incipient stage fire. In column F the Recordable case 1 on the Log were not descriptive, in that the body part affected, and/or the agent, object, and/or substance that directly injured or made the person ill was not noted/documented.  b.) Tonnard Manufacturing Corporation- The recordable injury and/or illness entries on the OSHA 300 Log for Calendar Year 2022 were not completed as required. The number of days the injured or ill worker was away was not recorded on the log.
