29 CFR  1910.107(b)(5)(iv):  Space within the spray booth on the downstream and upstream sides of filters shall be protected with approved automatic sprinklers:  At a facility located at W6365 Discovery Drive, Appleton, WI 54914; the employer did not protect the downstream and upstream sides of filters in the Upholstery spray booth with approved automatic sprinklers. Employees were engaged in spray application of flammable adhesives in the spray booth; automatic fire protection was not provided on either side of the spray booth's filters.

29 CFR  1910.107(b)(9):  Spray booths shall be so installed that all portions are readily accessible for cleaning. A clear space of not less than 3 feet on all sides shall be kept free from storage or combustible construction:  At a facility located at W6365 Discovery Drive, Appleton, WI 54914; the Upholstery spray booth was not installed so that a clear space of not less than 3 feet on all sides was kept free from storage or combustible construction. Storage along the sides of the booth, such as a flammable cabinet and steel racking, did not keep a clear space of 3 feet along the sides of the spray booth. Combustible construction, such as foam insulation pieces, were located on the top of the booth around the ductwork for the booth's ventilation system.

29 CFR 1910.303(b)(2):  Listed or labeled equipment shall be installed and used in accordance with any instructions included in the listing or labeling:  At a facility located at W6365 Discovery Drive, Appleton, WI 54914; metallic outlet boxes (handy box) were connected to flexible cords and used as suspended pendant outlets throughout the Upholstery shop. A pendant outlet near the Upholstery spray booth was used to power outlets in a work bench which were used to power a heat gun. The metal outlet boxes were not installed in accordance with the UL identifier QCIT, which states that boxes shall be mounted in or on a surface.
