29 CFR  1910.269(b)(1)(i): When two or more employees were performing field work on, or associated with, exposed lines or equipment energized at 50 volts or more, the employer did not provide at least two trained persons with first-aid training.  BIA COLORADO RIVER AGENCY ELECTRICAL SERVICES: On or about September 29, 2022, the employer did not have available at least two persons with first-aid training when a crew of four employees performed field work at the Poston Housing Development while working on, or associated with, exposed lines energized at 120 volts.

29 CFR  1910.269(d)(2)(vi):The employer did not provide training to ensure that the purpose and function of the energy control program were understood by employees and that the knowledge and skills required for the safe application, usage, and removal of energy controls were acquired by employees.  BIA COLORADO RIVER AGENCY ELECTRICAL SERVICES: Employees repairing a 120V electrical line were exposed to electrical hazards without having been trained in the employer's energy control procedures.

29 CFR  1910.269(d)(7)(ii):Notification. Affected employees shall be notified by the employer or authorized employee of the application and removal of lockout or tagout devices. Notification shall be given before the controls are applied and after they are removed from the machine or equipment.  BIA COLORADO RIVER AGENCY ELECTRICAL SERVICES: On or about September 29, 2022, affected employees were not notified by the employer of the reenergization of 120V electrical equipment where an employee sustained an electrical shock.
