29 CFR  1926.102(a)(1):Employees were not provided with eye and face protection equipment when machines or operations present potential eye or face injury from physical, chemical, or radiation agents:  On or about October 20, 2022, employees were exposed to a struck by hazard, while performing roofing work on a residential home located at 6827 Page Road, STREETSBORO, OH 44241. The employees were using air powered nail guns and the employer did not ensure that they were wearing any form of eye protection.

29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  On or about October 20, 2022, employees were exposed to a fall hazard of 9.6 feet while performing roofing activities on a residential home located at 6827 Page Road, STREETSBORO, OH 44241. Employees were using an extension ladder to access the roof and the employer did not ensure it extended at least 3 feet above the upper landing surface.
