29 CFR  1904.10(a): Where an employee's hearing test (audiogram) revealed that the employee had experienced a work-related Standard Threshold Shift (STS) in hearing in one or both ears, and the employee's total hearing level was 25 decibels (dB) or more above audiometric zero (averaged at 2000, 3000, and 4000 Hz) in the same ear(s) as the STS, the employer did not record the case on the OSHA 300 Log:  a) Simona America Industries- An employee experienced a work-related Standard Threshold Shift and the employer did not record the case on the OSHA 300 log, on or about October 17, 2022.   Abatement certification required.

29 CFR  1904.29(b)(1): A log of all recordable work-related injuries and illnesses (OSHA From 300 or equivalent), was not completed in detail as required by the regulation:  a) Simona America Industries- The employer did not list object or substance that directly injured or made the person ill, in column F for each employee listed on the 300 log for 2022.  The last injury documented before inspection was on November 26, 2022.   Abatement certification required.
