29 CFR  1926.100(a):Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:  On or about November 7, 2022, employees were exposed to a struck by hazard while performing roofing activities on a residential home located at 11550 Hoffman Road, Parma Heights, Ohio 44130.  Employees were working on the roof with tools and materials that could have fallen on the ground employees and the owner did not ensure that the ground employees were wearing protective helmets.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   On or about November 7, 2022, employees were exposed to a fall of approximately 18 feet while performing roofing activities on a residential home located at 11550 Hoffman Road, Parma Heights, Ohio 44130.  Employees were working on all areas of the roof and the employer did not ensure they were using fall protection.    BH Construction & Remodeling LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(10), which was contained in OSHA inspection number 1546461, citation number 1, item number 2, and was affirmed as a final order on 9/8/21, with respect to a workplace located at 16 Turrentine Street, Pittsburg, PA 15219.  BH Construction & Remodeling LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501 (b)(13), which was contained in OSHA inspection number 1486417, citation number 1, item number 2, and was affirmed as a final order on 9/24/20, with respect to a workplace located at 4949 Roscommon Drive, Brunswick, Ohio 44212.

29 CFR  1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:   On or about November 7, 2022, employees were exposed to a fall of approximately 8 feet while performing roofing activities on a residential home located at 11550 Hoffman Road, Parma Heights, Ohio 44130.  Employees were using an extension ladder to access the roof and the employer did not ensure it extended three feet above the landing area.  BH Construction & Remodeling LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.1053 (b)(1), which was contained in OSHA inspection number 1486417, citation number 1, item number 3, and was affirmed as a final order on 9/24/20, with respect to a workplace located at 4949 Roscommon Drive, Brunswick, Ohio 44212.
