29 CFR  1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   a. In the Tube bend area, on or about  November 15, 2022, and at times prior, the Tube bend Operator were exposed to  struck-by hazard. The employer did not  provide machine guarding  on the horizontal bandsaw blade, when cutting the metal tube to various sizes. There was no guarding on the bandsaw to prevent the employees from coming in contact with the engaged saw blade unused section, exposing employees to amputation injuries.
