29 CFR 1910.184(d): Each day before being used, the sling and all fastenings and attachments were not being inspected for damage or defects by a competent person designated by the employer:   a) Machine shop: On or about 8/19/2025, employees were exposed to struck-by hazards during material handling operations, including but not limited to the movement of bar stock. The employer did not ensure that a competent person inspected slings and their attachments for damage or defects prior to daily use, as required. The absence of a sling inspection program increased the risk of equipment failure during lifting operations, potentially resulting in dropped loads and serious injury to employees working nearby.

29 CFR 1910.215(b)(9): The distance between the abrasive wheel periphery(s) and the adjustable tongue or the end of the safety guard peripheral member at the top exceeded one fourth inch:  a) Machine shop: Baldor 248-1811D pedestal grinder with belt sander attachment: On or about 8/19/2025, employees performing grinding operations were exposed to struck-by hazards because the left tongue guard was not properly adjusted to the required 1/4-inch clearance; it measured approximately 2 1/2 inches.  b) Machine shop: DeWalt DW756 pedestal grinder, Serial Number 2014-26-YL-105581: On or about 8/19/2025, employees performing grinding operations were exposed to struck-by hazards because the left and right tongue guards were not properly adjusted to the required 1/4-inch clearance; the left measured approximately 1/2 inch and the right measured approximately 1 1/4 inches.
