On or about November 11, 2022, at the facility located at 338 South Main Street, Cedar Grove Wisconsin; the employer failed to ensure that altered or defective slings were not used.  Employees used a three-leg alloy steel chain sling with a master ring that was engraved with "Kuplex T/V" and "K-2 G8/G10" on the other side.  The sling had three hooks with "SN28266" engraved on the sling.  The "J" portion of the hooks were ground down to make a pins.

Employees used a three-leg alloy steel chain sling with a master ring is engraved with "Kuplex T/V" and "K-2 G8/G10" on the other side.  The sling did not have a permanently affixed identification.
