29 CFR 1910.184(c)(1): Slings that are damaged or defective shall not be used.  On or about November 11, 2022, at the facility located at 338 South Main Street, Cedar Grove Wisconsin; the employer failed to ensure that altered or defective slings were not used.  Employees used a three-leg alloy steel chain sling with a master ring that was engraved with "Kuplex T/V" and "K-2 G8/G10" on the other side.  The sling had three hooks with "SN28266" engraved on the sling.  The "J" portion of the hooks were ground down to make a pins.

29 CFR 1910.184(e)(1): Sling identification. Alloy steel chain slings shall have permanently affixed durable identification stating size, grade, rated capacity, and reach.  On or about November 11, 2022, at the facility located at 338 South Main Street, Cedar Grove Wisconsin; the employer failed to ensure that alloy steel slings used in the workplace contained permanently affixed identification stating size, grade, rated capacity, and reach.  Employees used a three-leg alloy steel chain sling with a master ring is engraved with "Kuplex T/V" and "K-2 G8/G10" on the other side.  The sling did not have a permanently affixed identification.
