29 CFR  1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At jobsite located at 2601 Central Ave, Dodge City, KS, an employee engaged in fire sprinkler removal activities was exposed to fall hazards of approximately 8.5 feet from bottom of the wooden frame support of the drop ceiling to the concrete floor below in that fall protection was not utilized.

29 CFR  1926.1053(b)(4):Ladder(s) were used for purposes other than the purpose for which they were designed:  At jobsite located at 2601 Central Ave, Dodge City, KS, an employee engaged in fire sprinkler removal activities was exposed to fall hazards of approximately 8 feet in that the employee climbed the 8 feet tall A-frame ladder to access the 8.5 feet high wooden frame support of the drop ceiling, exposing him to an eight feet fall from ladder to the concrete ground below.

29 CFR  1926.503(b)(1):The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  At jobsite located at 2601 Central Ave. Dodge City, KS, the employer did not prepare a written certification verifying that employees had been trained in hazards related to falls from elevation.  An employee engaged in fire sprinkler removal activities was exposed to fall hazards of approximately 8.5 feet from bottom of the wooden frame support of the drop ceiling to the concrete floor below in that fall protection was not utilized.
