29 CFR  1926.501(a)(2):Employees were allowed to work on surfaces which did not have the requisite strength and structural integrity:  At the commercial jobsite located in the KCI Max Building at 500 Reformatory Rd. Hutchinson, KS, an employee engaged in sprinkler installation services was exposed to fall hazards of approximately nine feet from the upper walking/working surface to the concrete floor below. The employee was standing on drywall and metal framing which did not have the structural integrity to support the employee.

29 CFR  1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  At the commercial job site located in the KCI Max Building at 500 Reformatory Rd. Hutchinson, KS, employee(s) engaged in sprinkler installation services was exposed to fall hazards of approximately nine feet from the upper walking/working surface to the concrete floor below in that fall protection was not utilized.   American Fire Sprinkler Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.501(b)(1), which was contained in OSHA inspection number 1638861, citation number 1, item number 1 and was affirmed as a final order on 5/02/2023, with respect to a workplace located at 2601 Central Avenue Dodge City, KS 67801.

29 CFR  1926.1053(b)(1): Where a portable ladder was used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.    At the commercial jobsite located in the KCI Max Building at 500 Reformatory Rd. Hutchinson, KS, employee(s) engaged in sprinkler installation services was exposed to fall hazards of approximately nine feet from the upper walking/working surface to the concrete floor below. The employees were using the top landing of the ladder to access the upper working surface. The ladder was not secured to the structure and a grasping device was not provided.   American Fire Sprinkler Corporation was previously cited for a violation of this occupational safety and health standard or its equivalent standard 1926.1053(b)(4), which was contained in OSHA inspection number 1638861, citation number 1, item number 2 and was affirmed as a final order on 5/02/2023, with respect to a workplace located at 2601 Central Avenue Dodge City, KS 67801.
