OSH ACT of 1970 Section (5)(a)(1):The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fall and struck-by hazards:   a) Between 96 Middletown Rd, Holmdel NJ 07033 and 64 Middletown Rd, Holmdel NJ 07033: Employees rode on the rear steps of a rear loading compacting vehicle when traveling between collection stops that measured approximately 0.54 miles apart.  This condition was noted on or about 12/02/2022.  b) Zone 5 collection route: On or about 12/2/22 an employee struck his face on a utility pole while riding on the back of a rear-loading compacting vehicle.  The road was a two-lane highway with no shoulder. The roadway was not identified as a hazardous travel route for employees riding on the rear step of the loader by the employer's safety program.  This condition was noted on or about 12/02/2022.
