29 CFR  1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used:  a) On January 18, 2023, and times prior training was not provided before having an employee work from a mobile scaffold that had  cracked components on the working platform.  This allowed the platform to bow downward under the employee.  The scaffold casters were not locked down, making the scaffold unstable as the employee moved around.  The employee was not trained to recognize these hazards or what measures were needed to correct the hazard.       Note) Certification of corrective action is needed for this item.
