29 CFR  1926.451(f)(3): Scaffolds and scaffold components were not inspected for visible defects by a competent person before each work shift:  a) On January 18, 2023 and times prior a pre-use inspection was not conducted prior to allowing an employee to use a rolling scaffold.  The metal trim on the 3/4-inch wooden scaffold plank was cracked in multiple places.  This allowed the platform to noticeably bow downward under the employee.   Note) Certification of corrective action is required for this item.

29 CFR  1926.452(w)(2): Scaffold casters and wheels were not locked with positive wheel and/or wheel and swivel locks, or equivalent means, to prevent movement of the scaffold while the scaffold was used in a stationary manner:  a) On January 18, 2023, an employee worked from a mobile scaffold without all the wheel casters in the locked position.  The scaffold was not stable as the employee moved around on it.    Note) Corrected during inspection.

29 CFR  1926.454(a): The employer did not have each employee who performs work while on a scaffold trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used:  a) On January 18, 2023, and times prior training was not provided before having an employee work from a mobile scaffold that had  cracked components on the working platform.  This allowed the platform to bow downward under the employee.  The scaffold casters were not locked down, making the scaffold unstable as the employee moved around.  The employee was not trained to recognize these hazards or what measures were needed to correct the hazard.       Note) Certification of corrective action is needed for this item.
