29 CFR 1926.451(f)(14): Makeshift devices, such as boxes and barrels were used on top of scaffold platforms to increase the working level height of employees. a) On March 21, 2023, an employee engaged in drywall activities was exposed to fall hazard while working from approximately 10 feet above the walking surface on a makeshift wooden platform supported by the mid-rails of a scissor lift. Note) Abatement certification is required for this item.

29 CFR 1926.502(d)(15): Anchorage used for attachment for personal fall arrest equipment was not capable of supporting at least 5,000 pounds (22.2 kN) per employee attached nor was it designed, installed and used as follows: As part of a complete personal fall arrest system which maintains a safety factor of at least two; and under the supervision of a qualified person. a) On March 21, 2023, an employee engaged in drywall activities was exposed to a 10 foot fall hazard to the walking surface while tied off to an anchorage point not designed to support 5,000 pounds per employee. An employee tied off to a 60 to 72-inch long 12-gauge solid strut channel with a maximum allowable uniform load of 560 to 680 pounds Note) Abatement certification is required for this item.
