29 CFR  1910.25(b)(4):29 CFR 1910.25(b)(4): The employer did not ensure that stairway landings and platforms are at least the width of the stair and at least 30 inches (76 cm) in depth, as measured in the direction of travel.  Instance 1: outside stairway to the back of the head saw. on or about 1/25/2023, the stair top landing was only 10 inches deep, exposing employees to a fall hazard.

29 CFR  1910.140(c)(18):29 CFR 1910.140(c)(18): The employer did not ensure that personal fall protection systems are inspected before initial use during each work shift for mildew, wear, damage, and other deterioration, and defective components are removed from service.  Location: head saw On or about 1/24/23, a retractable lanyard used when changing out the head saw band saw blade was not inspected to ensure it was working properly.  The lanyard would not retract and used with 7 feet of the lanyard hanging down.
