29 CFR 1926.300(b)(1): Power operated tool(s), designed to accommodate guards, were not equipped with such guards when in use:  a) Jobsite located at 1238 Buttonwood Street, Philadelphia, PA. - The employer failed to ensure that all hand operated power tools, including a DeWalt DWE402 4-1/2 inch Angle grinder operating at 11,000 RPM, was equipped with a guard where a guard was designed to be mounted, thus exposing employees to the rotating wheel.   Condition was observed on or about February 1, 2023.

29 CFR 1926.451(b)(1):Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii):  a) Jobsite located at 1238 Buttonwood Street, Philadelphia, PA.- The employer did not ensure that the working level on the supported tubular welded frame scaffold used by an employee to weld steel beams was fully planked, thus exposing the employee to slip and fall hazards of approximately 6 feet.  Condition was observed on or about February 1, 2023.
