29 CFR  1926.501(b)(10):  Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:   a) Jobsite, 611 Woodlawn Road, Aliquippa, PA 15001:  Observed on or about April 25, 2024, four employees installing metal panels on the roof of a one-story building with a low slope roof were not protected from falling.  The employees were exposed to a fall  hazard of approximately 10 feet 10 inches to the ground below.

29 CFR  1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer:    a) Jobsite, 611 Woodlawn Road, Aliquippa, PA 15001:  Observed on or about April 25, 2024, the employer did not maintain a written (training) record of fall protection for each employee who was installing metal roofing panels to the roof of a one story building.
