29 CFR  1910.22(b): The employer did not ensure that each walking-working surface can support the maximum intended load for that surface.  On or about March 9, 2023, the employer did not demonstrate that structures were safe as walking working surfaces due to corrosion, exposing employees to struck by, fall and trip hazards in the following areas, but not limited to these instances:  a. Employer did not provide a safe walking working surfaces in the wet pallet area adjacent westside stairs, in plant 1, due to a support beam for the floor having the webbing rusted completely out due to corrosion.    b. Employer did not provide a safe walking working surfaces in the leveling screw area adjacent westside stairs, in plant 1, where  a hole in the floor, in the front of the leveling screw machine,  caused by corrosion of the metal exposed employees to trip hazards.  c. Employer did not provide a safe walking working surfaces in the PO2 Spoon area, in plant 2, due to holes in the grating of the walking working surfaces exposing employees to trip hazards due to the corrosion.  d. Employer did not provide a safe walking working surfaces in the conveyor area, in plant 2, due to a hole in the walking working surfaces caused by the corrosion, exposing employees to trip hazards.  e. Employer did not provided a safe walking working surfaces in the C32 area, in plant 2, where is a hole in the walking working surface, exposing employees to trip hazards due to the corrosion. The grating and the support for the grating was corroded and part of the grating had separated due to the corrosion.  f. The employer did not ensure that stairs near the twilight tank area, in plant 2 can support  anticipated live load exposing employees to trip hazards due to the corrosion and holes in the stairs walking working surface.    g. The employer did not ensure that stairs near the weigh tank, in plant 2 can support anticipated live load exposing employees to trip hazards due to the corrosion and holes in the stairs walking working surface.

On or about August 29, 2023, the employer did not ensure stairs had uniform riser heights exposing employees to trip hazards in the following areas but not limited to the stairs in plant 2, line 3 leading up between the north and south unit 2 reject tank, the risers on the second step from the ground were bent and damaged .

On or about March 9, 2023, the employer did not ensure that the midrails of guardrail systems were capable of withstanding, without failure on the east catwalk adjacent C42 near weigh area, in plant 2 but not limited to, exposing employees to falls hazards due to the corrosion of railings and support structure for the railings. Pieces were missing due to corrosion and sections were separated from each other due to corrosion.   b. On or about, and times prior to August 29, 2023, the employer did not ensure that the midrails of guardrail systems were capable of withstanding, without failure on the east catwalk adjacent C42 near weigh area, in plant 2 but not limited to, exposing employees to falls hazards due to the corrosion of railings and support structure for the railings. Pieces were missing due to corrosion and sections were separated from each other due to corrosion.

The employer did not ensure that handrails and the top rails of stair rail systems are capable of withstanding, without failure, a force of at least 200 pounds (890 N) applied in any downward or outward direction within 2 inches (5 cm) of any point along the top edge of the rail.   a. On or about March 9, 2023, the employer did not ensure that top rails of stair rail systems were capable of withstanding, without failure, on the mixing area west stairwell landing, in plant 1 but not limited to, exposing employees to falls hazards due to the corrosion of railings and support structure for the railings. All structures in the area were not maintained according to the standard. Pieces were missing due to corrosion and sections were separated from each other due to corrosion.   b.  On or about, and times prior to August 29, 2023, the employer did not ensure that top rails of stair rail systems were capable of withstanding, without failure, on the mixing area west stairwell landing, in plant 1 but not limited to, exposing employees to falls hazards due to the corrosion of railings and support structure for the railings. All structures in the area were not maintained according to the standard. Pieces were missing due to corrosion and sections were separated from each other due to corrosion.   c. On or about, and times prior to August 29, 2023, the employer did not ensure the top rails of the stair rail system in plant 2 but not limited to, the stairs from ground to mixing area on southside, was capable of withstanding minimum applied outward direction force, exposing employees to fall hazard due to damage of stair rail system.
