29 CFR  1926.451(b)(2):Scaffold platforms and walkways were not at least 18 inches wide:  On or about: April 20th 2023 Location: 331 Tiffany Place, Bronx, NY 10474  a) Employee working on an elevated platform fell to the ground when employee hit a j-bolt that was holding the Doka formwork walers. J-bolts were protruding into the elevated platform in the area where the employee fell down leaving some sections of the platform with only  13 inches wide of clearance.
