OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by and crushing hazards:  a) Worksite, 18 Main St., Wappingers Falls, NY, dumpster:  On or about February 13, 2025  an employee was unhooking a debris bag from a TAG Manufacturing Inc. bucket attached to a WainRoy Manual release coupler that was attached to a Volvo EWR 150 wheeled excavator. The square locking pin unfastened from the coupler causing the latch pin and bucket to fall off the excavator. The bucket struck and injured an employee. The square locking pin used was not of the type the manufacturer called for and it did not have a ring that locked it in place over the latch pin.
