29 CFR 1910.29(b)(13):  The employer did not ensure that when guardrail systems are used around holes that serve as points of access (such as ladderways), the guardrail system opening meets the requirements listed in 29 CFR 1910.29(b)(13)(i) or 29 CFR 1910.29(b)(13)(ii):   (a)	Throughout the facility; Guardrail systems used around openings that serve as points of access for fixed ladders were not equipped with a self-closing gate that slides or swings away from the opening on ladders located in areas including but not limited to, the loadout bins, the mezzanine, and the main leg.

29 CFR 1910.272(j)(2)(ii):  The employer did not immediately remove any fugitive grain dust accumulations whenever they exceeded one-eighth inch (.32cm) at priority housekeeping areas, pursuant to the housekeeping program for grain elevators:  (a) Area below mixer; Fugitive grain accumulation greater than 1/8 inch in depth was present on the floor within approximately 20-feet of the boots for the mixing and receiving legs (bucket elevator) and on the floor of the pit in the immediate vicinity of the boots.

29 CFR 1910.303(g)(1):  Sufficient access and working space was not provided and maintained about all electric equipment (operating at 600 volts, nominal, or less to ground) to permit ready and safe operation and maintenance of such equipment:  (a)	Compressor room; The space in front of and adjacent to electrical service disconnects used to supply power for the Sanborn and Sullair brand air compressors was not maintained free of obstruction to allow for unrestricted access to the equipment's energy sources.
